Request An Appointment
You can email us with a request for an appointment. From this email, our receptionist will be able to review the information and search for an appointment time that best matches your request. The receptionist will then contact you in the method you indicate whether it be by phone, fax, or email.
Our scheduling software is not connected to the internet or this website in order to avoid any potential security threats. Be assured that your personal information is protected and not available to be hacked into through this website!

- Phone number
- What kind of appointment? (checkup and cleaning, filling, second opinion, etc.)
- What day of the week and what time do you want?
- How should we contact you? (Email or phone call)
